I don’t know much about ‘Universal 20W’ engravers but this looks exactly like the problem caused when a cog-belt slips and jumps one or two teeth (on one occasion) in a job. A loose belt can oscillate and make the belt jump or buck in inconsistent, seemingly random or unpredictable ways. What might your instruction manual say about belt tensioning?
