Any Idea what would cause this?

I wanted to make a simple template for my coasters. Set it up all nice and even. All perfect 4" circles.
Framed it then started it. First cut looked good. Turned my back to work other stuff then check it a few minutes later and saw this. What in the world?

hmmmm, check for loosen belts, it seems to me that your machine loose steps in both axis
